Per standing board policy, leaders of all designated Interest Groups which include active, outdoor activities are to confirm that we have signed liability waivers from all participants.
The following procedure should be used by leaders to implement this policy:
- Prior to the activity, the leader should review the waiver repository to verify that each participant expected has a waiver on file with the Club. This can be done easily online, or by using the Google Docs app on a mobile device.
- For any participant who does not have a waiver on file, the leader should have them complete the current form, including name, signature, date and emergency contact information.
- When convenient, photograph or scan the signed form (as either a PDF or JPG format image) and email it (full size) to email@example.com. Adding the member’s name in the subject would be helpful. Insure that the image fully reproduces the form (e.g. it is in focus and shows the entire form).
The group leader should retain the original paper form through the current program year, and can then be destroyed.
